Please note that the following documents are required for entrance into the program. If you are selected to participate you will need to provide these documents upon acceptance to the program. Please read carefully and contact us if you have questions – we would be happy to help.

1. Two pieces of identification (one must be photo ID) 

Acceptable forms of ID include:

  • Birth Certificate
  • Passport
  • Permanent Resident Card
  • Canadian Citizenship Card
  • Immigration Papers
  • Driver’s licence
  • Indian Status Card
  • Hospital Card
  • Age of Majoriity Card

 High School student cards and health cards are not accepted. Please check dates on cards to make sure they are still valid.

2. Social insurance card

3. A police record check form, filled out

Important: Please note that we only require the form to be filled out. If you are selected to participate, we will submit your police records check form for you at no cost. Do not submit your form to the police station.

4. A void cheque or bank deposit slip

If you don’t already have a bank account you will need to get one in order to provide a cheque or bank deposit slip.

5. Photo permission form

Unless you are 18 years or older your parent or guardian needs to sign this form.

6. Waiver Form

Unless you are 18 years or older your parent or guardian needs to sign this form.

7. Resume

Please provide the most current version of your resume.

8. Technology

Please ensure you have access to reliable technology (ex: Microphone and Webcam) and internet to participate in the program. If this will be an issue for you, please contact us at
